1. Acceptance of Terms

By subscribing to or using SureVisit Health, you agree to these Terms and Conditions and our Privacy Policy. You represent that you have the authority to bind your medical practice to these terms.

2. Medical Disclaimer

SureVisit Health is a scheduling tool, not a medical device. * Emergencies: The platform must not be used for emergency medical bookings. Patients experiencing an emergency must be directed to call 911.

  • No Medical Advice: SureVisit Health does not provide medical advice, diagnosis, or treatment.
     

3. Subscription Tiers & Predictable Pricing

  • Plans: Fees are based on your selected tier (Solo, Small Group, or Medium Group).

  • No Hidden Fees: Our pricing is fixed and predictable. We do not charge per-appointment or per-messaging fees.
     

4. Practice Responsibilities

  • BAA Requirement: You must execute a Business Associate Agreement with us before processing PHI.

  • Accuracy: You are responsible for ensuring provider availability and scheduling logic are accurate.

  • Verification: While the Browser Bridge is designed for high accuracy, it is your staff’s responsibility to verify data synced into your EHR before saving a record.
     

5. Intellectual Property

SureVisit Health, including its code, design, and the “Browser Bridge” technology, is the proprietary property of SureVisit Health, LLC. Drawing on over 25 years of design experience in high-volume healthcare environments, the platform is optimized for efficiency and is protected by copyright and intellectual property laws.

6. SMS Appointment-related Text Message Notifications

SureVisit Health provides appointment‑related text message notifications on behalf of participating health care practices. Message frequency varies based on upcoming appointments. Message and data rates may apply. Messages are transactional and non‑marketing. No protected health information (PHI) is sent by SMS.

Patients can opt out at any time by replying STOP. Opting out stops all text messages from the practice. Patients may opt back in by replying START. For help, reply HELP.

By providing a mobile number and completing the scheduling process, patients give express consent to receive appointment‑related text messages. Consent is not required for care.

7. Limitation of Liability

To the maximum extent permitted by law, SureVisit Health, LLC shall not be liable for any indirect or consequential damages. Our total liability shall not exceed the amount paid by you for the service in the 12 months preceding the claim.
